We have reported the case of a young boy with pyomyositis of the tensor fascia lata. Although pyomyositis is a rare disease, rapid diagnosis with MRI is essential for these patients. Treatment is based on appropriate antibiotherapy.

Introduction. Tensor Fasciae Latae (TFL) and the iliotibial band (ITB) act as a single functional unit in maintaining pelvic stability whilst standing, walking, or running in human beings. The Tensor Fasciae Latae (TFL) muscle acts across the hip and the knee joint.

We have reported the case of a young boy with pyomyositis of the tensor fascia lata. Although pyomyositis is a rare disease, rapid diagnosis with MRI is essential for these patients. Treatment is based on appropriate antibiotherapy.

The pattern of edema on MRI is similar to that of dermatomyositis and characteristically is symmetric bilaterally with a predilection for the tensor fasciae latae muscle and relative sparing of the adductor muscles .

MRI revealed increased signal intensity and hypertrophy of his left tensor fasciae lata muscle and atrophy and fat infiltration of his right tensor fasciae lata muscle (Fig. 1a). Increased signal was also seen in both gluteus minimi muscles on T2 weighted images.
